
A Family Partnership Meeting (FPM) is a structured approach to involve children/youth and families in decision-making. It is an opportunity for the family, their identified relatives, and natural supports to plan for the safety and permanence of their children/youth.
FPMs are strength-based, trauma-informed, and solution-focused team effort. FPMs are coordinated and facilitated by a trained professional. You will be provided guidance, information, and resources to ensure effective and active participation prior to the FPM. An agenda is followed and a written service plan is developed. Services are identified, specifics tasks and responsibilities are assigned to the family, their natural supports and service providers.
Children/youth and their parents/caregivers are encouraged to work with their assigned Social Worker, Kinship Navigator, and Family Support Coordinator to explore and identify relatives, natural supports, and service providers to participate at the FPM. You and your family’s active participation and voice are important throughout this process. Children are welcome to attend and participate when and if appropriate.
