The following is a list of available resources for adults in and around Arlington County for your convenience. Listing of any non-County resources does not imply an endorsement by Arlington County. They are simply for the convenience of our community members in need of non-emergent medical care, which is not appropriate for 911 emergency medical services.

Nursing Case Management (NCM)

The NCM program helps individuals improve or maintain their health so they can remain at home safely for as long as possible.
Case management and coordinating health care
Assessing and monitoring your health status
Pre-pouring your medications
Providing health education
Referring you for other services you might need

Qualifications

Arlington residents over 60 with multiple chronic illnesses
Adults 18-59 with permanent disability who:
require assistance
lack sufficient support
have a physician to order services
have no access to another source
no longer eligible for skilled care
