Support Coordination Services

Support Coordination services are activities designed to assist an individual in accessing needed medical, psychiatric, social, educational, vocational, residential, and other supports essential for living in the community and in developing his or her desired lifestyle.

Examples of Support Coordination services include:

  • Coordinating initial and annual assessment of the individual
  • Assisting an individual to secure services
  • Developing an individual’s Person-Centered Plan (PCP) through a team meeting
  • Coordinating and planning services and supports with other agencies and providers
  • Linking an individual to services and supports specified in the PCP
  • Assisting an individual directly for the purpose of locating, developing or obtaining needed services and resources
  • Enhancing community integration through increasing an individual’s community access and involvement
  • Making collateral contacts to promote implementation of the PCP and allow the individual to participate within activities in the community
  • Monitoring the implementation of the PCP through regular contacts with service providers, as well as periodic site and home visits
  • Monitoring the quality of services and individual satisfaction with services

Virginia Medicaid Waiver

Virginia offers multiple Waivers, including three specifically intended to serve children and adults with developmental disabilities.  The Virginia Developmental Disabilities Waiver Program provides Medicaid funding to qualified individuals to cover a continuum of “community-based” services including, but not limited to: group home residential/residential support services, in-home support services, group day support, group or individual supported employment, crisis supports, therapeutic behavior consultation or private duty nursing/skilled nursing.
