Program of Assertive Community Treatment (PACT)

 

Mission

The PACT Team mission is to provide a holistic community-based array of services to people with severe and persistent mental illness. The PACT Team utilizes a multi-disciplinary approach in a highly diverse community, with the intention of consistently providing high quality services that are specifically designed to meet the acknowledged needs of our consumers.

Services

Community mental health services provided by the PACT Team are:

  • 24-hour, 365-day-a-year services
  • Case management
  • Mobile outreach
  • Visits to consumers
  • Psychiatric services

Supportive services include budgeting, grocery shopping, transportation, activities of daily living, housekeeping, social skills development, vocational services, nursing services, and short-term, brief focused therapy.

Eligibility

Eligibility for PACT services includes:

  • Arlington County resident, 18 or older;
  • Severe and persistent mental illness: AXIS I diagnosis of schizophrenia, schizo-affective disorder, bipolar disorder, or other psychotic disorder;
  • History of multiple hospitalizations;
  • Difficulty managing many spheres of independent living such as housing, food preparation, home maintenance, finances, employment, and/or physical illness.
