
This medical model of Case Management is an Arlington County funded program that provides Nursing Case Management to older adults and persons with disabilities. An initial home visit is made by the Registered Nurse for a comprehensive assessment of health and wellness needs. The Nurse Case Manager visits monthly and as needed for medical monitoring, health education, coordination of care and linking with resources. Also offered as a part of Nursing Case Management is assistance by Human Service Aides with bathing and personal care one or two times a week. The fee for the bathing service is based on the patient's income.
Nursing staff offers educational programs for seniors and caregivers at a variety of sites throughout Arlington. The focus of programs are prevention with the idea of providing individuals with the information and resources they need to safely manage their health care issues independently in their own homes.
The Cluster Care Program provides eligible adults with long-term maintenance or support services enabling individuals to remain at or return home after hospitalization or skilled facility discharge. Services may include personal care services, nutritional support, environmental maintenance, and support services such as grocery shopping & running errands. Services provided are not medical in nature and therefore are not reimbursed by Medicare, Medicaid, or most health insurances.
