The 8 documents listed are considered the "Background Packet". You must bring this completed and notarized packet of documents with you in order to be admitted into the test site.
All forms are in PDF format and require the use of Adobe Acrobat 5.0 or newer. Forms can be completed within the PDF document and printed for submission to Police Human Resources. Unless you have the Acrobat writing software, you will be unable to save your completed documents for electronic submission. All of these forms are required to be completed for employment consideration at the Arlington Police Department after the initial phases of employment selection (for officers: the exam, for civilians: an interview).
All forms must be typed.
